Astria Home Health and Hospice completed a week-long CHAP Survey, with a perfect score, zero deficiencies. The Community Health Accreditation Program, Inc. (CHAP) is an independent, non-profit accrediting body, which was established in 1965.
CHAP accreditation publicly certifies that an organization has voluntarily met the highest standards of excellence for home and/or community-based health care. This survey audits and evaluates various components of the programs, including review of policies and procedures, auditing of Medical Records, and home patient evaluations conducted with Astria’s Clinical Staff. The survey is to ensure the organization complies and follows all CMS Medicare/Medicaid COP (conditions of participation) as well as, provide care to the community at the highest standards.
In order to receive zero deficiencies in a survey is a true testament of the compassionate quality care Astria Home Health and Hospice staff provide to the patients of this community.
