Welcome to your Astria Health Patient Portal! The patient portal is a secure, online home for your Astria Health medical information. Accessing your information requires simple log-in steps. In the patient portal, you can:

• View and share your personal health information related to your hospital stays and clinic visits
• Request an appointment with an Astria Health provider
• Have electronic access to your list of medications, diagnoses, allergies, lab test results, patient history and other care information.
• Securely send or share information electronically
• Access your hospital discharge instructions for better after-discharge care
• Manage family members' health information, if authorized

HealtheLife Patient Portal is available at no charge to you. You can also download a mobile application on your phone so that you have access to your health information all the time. The application is available for both IOS and Android devices.

If you have not yet signed up for a patient portal, contact your care provider. Then, simply follow the steps shared with you via email. Please note that you will need to have the security question information given at the time of registration ready when you log-in. Once you have registered and created your log-in information, simply click below to login.
