Astria Home Health is able to bring quality medical care directly to your home through skilled nursing, physical therapy and other care services.

Astria Home Health works with you, your loved ones and your doctor to develop a plan of treatment that meets your physical and emotional recovery needs. Our work is successful when we have exceeded your expectations.

Your Care Team

Our outstanding staff is committed to compassionate service and the role they play in your home healthcare. Your care team will be comprised of:

  • Nurses (RNs or LPNs)
  • Physical and speech therapists
  • Medical social worker
  • Home health aides

Accepted Forms of Payment

At Astria Home Health, we want for you to be able to receive the care you need without financial worry. We accept private pay and other insurance and, for those who qualify, Medicare or Medicaid pays for 100 percent of your care.

Hospice Care

Astria Health Hospice staff and volunteers are dedicated to providing loving care and support to families and patients with life-limiting illnesses. Our hospice team offers comfort care, pain and symptom relief, and emotional and spiritual support. Families of hospice patients are provided with arrangements for respite care and bereavement services. Our staff has been caring for families for more than 25 years. Hospice is a service that is completely covered by Medicare, Medicaid and most private insurance providers.

Our multidisciplinary team provides medical care, education and emotional support through services such as:

  • Professional nursing care and home visits
  • Pain and symptom management
  • Hospice aides services (bathing, dressing)
  • 24-hour on-call service
  • Continuous care (for brief crisis periods)
  • Respite care
  • Medication and medical equipment for home care
  • Nutritional counseling and education
  • Medical social work services
  • Spiritual care
  • Bereavement support
